Happened to someone I know when he was playing GTA: SA. You just need to call Sony customer service and explain the problem to them. You'll probably have to ship the PS2 to Sony to have it fixed.

it could be that the removeable media drive is going out or the laser on the drive needs to be adjusted. it happens with every DVD player and drive that i've owned. you may be able to do a google search on cleaning or adjusting your PS2 DVD drive and find some good tips.
on the simple side, it may just be that the disk needs to be cleaned. but i'm sure you've already checked that... so...
