This is actually a good question, because all the shuttle deaths we heard about occurred while going into or down from space. However, in 1971, three Russian cosmonauts of Soyuz 11 were killed by exposure to vacuum after undocking from space station Salyut 1 when a valve accidently opened. Their craft subsequently landed normally, but a recovery crew discovered their dead bodies.

No. The astronauts of Challenger and Columbia died in the Earth's atmosphere, as did those of Soyuz 11. The Apollo 1 astronauts died on the Earths' surface in a fire, and Soyuz 1 only killed its pilot when the craft crashed to the ground. So although humans have died on space missions, none of the fatalities actually occurred in space.

I know one dog died in orbit....lifesigns from the dog went out at 71 miles...but thats a dog...poor thing.
