Turn off the water supply to the toliet.
Use an old towel to remove any excess water.
Loosen / remove the bolts at the base.
Lift off old tank.
Replace seal and install new tank.
Turn water back on and check for leaks.
Put tools away and have a beer.

Turn off the water supply (Valve is under the tank)..Flush the toilet as normal but this time hold down the lever for a couple of minutes, (This allows as much water as possible to drain from the tank) Remove the remaining water at the bottom of the tank with a towel. Disconnect the water supply line. Remove the two nuts that secures the tank. Remove and replace the 3 inch ring. While you got it off you should replace it with a new one. Replace the tank making sure that the nuts are EVENLY secured. Replace the water supply line and turn the water back on. Sit on the toilet and complete two very difficult sodoku puzzles, (diabolical ones). After your legs go to sleep, have someone help you up and lie on a bed till you get circulation back in your legs. Go back and check below the tank to see that there are no leaks. If no leaks..... you're done. Congratulations!

I'm surprised with all the answers, it doesn't sound like anyone has ever really done it. It is quite simple, and it's a good idea to replace the gasket but the most important thing is to screw each one of the bolts just a little at a time working your way around over and over in order to not apply too much pressure at any one area causing the tank to crack.

Shut the water off to the tank, flush it and I usually take a bath towel to get the excess water out of the tank. You will be able to see the screw heads, just unbolt it and lift it off. Make sure you use a new gasket when you put the new one on.

turn off the water. flush to remove most of the water in the tank. unbolt, unfasten water supply, lift, remove. place new tank, bolt in place, reattach water supply, turn water on.
