There have been quite a few accidents involving blackhawk helicopters, but I am not sure if they are the worst. The Sea King is getting pretty bad but this is related to issues with the age of the airframe. Blackhawk accidents have been mainly due to pilot error and may not be a reflection of the safety of the aircraft but problems with training and experience. Also there are a lot of blackhawks around so statistically it may not be that bad. Out of interest Australia just lost a blackhawk about 2 hours ago off the coast of Fiji.

No, its the Sea King (Canada) if you're talking military only. Those Helicopters are more than 25 years old.
