You first should have made it aware at the establishment that the pizza came from. If the manager or owner was not cooperating, then you should call the better business bureau. If you are complete put off, then skip step one and go straight to step two. You can also call the health dept.

It makes no difference really whether it was alive or dead- if you ate it, you digested it. The stomach acids will eat through bone, so no little bug will survive that.
You watch "survivor" or "castaway" on TV? they eat much worse and live. In many cultures bugs are the mainstay of diet (or rather the only protein available)
A different thing altogether is the legal aspect- i.e. the responsibility of the manufacturer/ seller. Obviously his storage facilities are not adequate. IMO talk to them- provided you want to eat anything from their shop again. if not, call the sanitary inspectors.

Take a photograph of the pizza with worm and let other people witness what you did. File a formal complaint against the pizza company so that they will be careful next time about their food preparation.

You will not be able to prove much unless the thing was spotted on the pizza when you were taking it from the pizza shop.at least you cannot say their meat is not fresh.
