Only one Royal Rumble was on Cable. That was the very first Royal Rumble on January of 1988 that was shown on the USA Network. Jim Duggan won that Royal Rumble eliminating One Man Gang(Akeem). Since that time every Royal Rumble has been on PPV.

For the most part yes. But every once in a while you will get one, but it won't be as good. Kind of like the one that Eddie won a few weeks ago, there has also been the corporate Rumble, and the first ever Rumble was on TV, but these are very rare occasions.

Not sure how it is in the States. Whenever they show a special wrestling program here in Germany, like Royal Rumble, Wrestlemania, etc., it costs the equivalent of 17 US dollars. I think that's pretty expensive!

All of them are PPVs... duh. Unless you want to wait for them to come out on DVD or learn how to use Azureus to download them (if you can find any torrents).
