MLA Format Cover Page?

Question

I need a cover opage for my research project. I thought it would be my name, teacher, class, date, and title like on any other MLA page, but seperate and on the cover, but apparently there is a published cover page in MLA format avilable, and APPARENTLY it is in the LBHB by Ramsey Fowler 10th edition but I cannot find it anywhere. Please help! (and dont tell me there isnt a mla cover page format, because I knwo there is one.. its in there, I just cant find it or anyhting online about it, because all i find are mock examples the teachers mad eup for their class)

The OWL at Pardue only states how to add the cover to any regular page. The last name, teacher, class, and date, with the title centered are basic outline for a regular page, known as the header. It is NOT the cover page.

I have foudn it in the book, page 690 if anyone is interested. 1/3 of the page down, you put the title, all centeres. Onc einch down from that, BY, enter once and type the name. Ince inch from that down, the teachers name, enter, course, enter, and date in formate DAY MONTH YEAR.

It doesn't exist. The OWL at Purdue expressly states this on the first page. Check my source. In short, all you do is put your last name and pg # in the top right corner. then, double spaced you put

Your Name

Teacher's Name

Course

Date Due of Paper

(centered) title

(indent 5 spaces) start paper
