I think it's just called a play within a play. The French term is: 'Mise en abyme'

It's when people (or kids) pretend to be a doctor so they can tell their "patient" to undress and touch parts of their body, etc. It's sexual in nature.

a doctor of divinity would be a preacher, maybe a priest, bishop, cardinal, etc.
