Frumpy always has a negative connotation. It doesn't necessarily imply obesity or laziness...it describes someone (almost always a woman) who is lacking grace, untidy in what she wears, not looking after her appearance very well. Doesn't necessarily indicate laziness, either... a hardworking maid whose hair is messy (perhaps from her exertions), whose clothing is unflattering (maybe some sort of shapeless unfeminine working clothes) is just as frumpy as a woman of similar appearance who is just a lazy couch potato.
It doesn't imply anything gross like never bathing, or any major sort of hygiene problem. Just that they don't look like they make any effort to make themselves as attractive as they could be.

Frumpy doesn't necessarily mean "fat" - although that can definitely be a component of it - a thin person can be frumpy if they dress eclectically, old fashioned, a bit rumpled - yes, comfortable (not really lying around though, that's a verb, frumpy is definitely an adjective).

Frumpy:
Granny glasses attached to a pearl chain worn low on the nose, hair tossed into a messy bun, too-large cardigans with a moth hole or two over a mid-calf easy flowing skirt, carrying a canvas book bag with second hand books and maybe some veg peeking out the top, mismatched (but not badly) wool socks worn with old sneakers, probably followed by a friendly cat. Perhaps a few pounds too many, perhaps not. Perhaps young, perhaps not. That's frumpy.

I agree, it doesn't have to mean fat. Mary Kate Olsen often looks frumpy. It means kind of disheveled, not very stylish, kind of "homeless" looking. Dull. Usually referring to women.

I usually just think of it as sloppy and unkempt.
