The Chernobyl disaster had two root causes. First, the reactor had a design flaw. The safety mechanisms were powered by the reactor itself rather than an independent power supply. Second, the safety mechanisms were systematically disabled by an engineer intent on testing the generator that powers the emergency shut down. I attended a symposium given by an IA EC scientist shortly after the event. He likened the human error at Chernobyl to opening an aircraft door at 30,000 feet. The aircraft operating manual does not specify that the door should not he opened at altitude because it is assumed that no one would be stupid enough to try. Chernobyl is a monument to human stupidity.

This is a good place to start. It is an account of some journeys made by a girl with a motorbike through what is left of Chernobyl. Parts of her account are chilling.
