Why doesn't it appear on all the channels?

Usually appears as the adverts are about to come on.

I bet its followed closely by an advert break or the end of the show

It's the bar code for end of the program or advert break

If it's what I think you are asking about, all I know is that it indicates the program is about to end or go to the adverts.

Well, it seems to indicace when a break, or the end of the program, is on it's way.
