Max flue length that I am aware of is 3m on a Vaillant. Should also be RSFA (Room sealed fan assisted) Balanced Flue.

An outside wall yes! A combi uses a balanced flue, that is it sucks air in and blows fumes back out at the same time, there are many regulations surrounding gas appliances and you would be getting the thing connected by a gas installer anyway. It cant be placed too near a window or close to a conservatory etc as the fumes could enter the building through an open window.

yes, but nearly all boilers fitted have to be H.E. (high effeciency) unless you follow the exceptions procedure and acrue enough points to fit a standard efficiency. A H.E. boiler has a condensate pipe that requires plumbing into a waste or soil pipe, or in some cases you could run it into a drain pipe or a soak away. The difference between H.E. and S.E. is that the H.E. boiler will potentially save you some money and be greener to the environment. As for the length of flue req'd, every boiler has different limitations and this can be checked with the manufacturers fitting instructions. Also alot of manufacturers now produce pluming kits to make it easier to disperse the plume away from windows, doors or the next door neighbours property as pluming can cause a nuisance. If in doubt, always consult the manufacturer.

Not an expert, but have a combi at home. When I got it fitted, the choice of location was mine. For convenience and minimal plumbing costs it was easier to put it in the bedroom closet. It is very silent and the flue vents outside. However since then I got married and my wife has started to speak gibberish, I now believe that it should be plumbed away from the bedroom :-)

Yes. The important consideration is the position of the flue outlet, and, yes, I believe you will find 3m is the maximum recommended length for a horizontal flue, although it is possible to buy balanced flue boilers with a vertical flue to go out of a roof.
