Here's a great idea...I haven't let my secret out, but I guess since your in a pinch try this. First, explain that you made a mistake and thank him for setting another appointment. Next, go to a local candy store and have them make you a foot size chocolate foot. Yes, a chocolate foot. Take it to your interviewer with a note that says "Just trying to get my foot in the door". It never fails. They remember you and along with a good impression you should get the job within a day or two.

Yeah, I think that's going to impact negatively on your interview. It kinda shows that you weren't paying attention when you made the appointment. All you can do it apologise for the mistake, say that that has never happened to you before (but don't overdo it) and hope they ignore it. Try to draw attention to your other qualities, instead.
In future, always check the date and time with the person who makes the appointment, and if it's more than a week away, always call the day before and make sure the appointment is still good.

Don't blame anyone. Even if it was someone else's fault, you don't want to seem like you are trying to pass the buck or point the finger.
I would simply state that there was a miscommunication and you apologize for any inconvenience.
Don't spend the whole interview apologizing. Once they accept your apology, move on with the interview and don't come back to it.

it is okay that this happened. you will get yet another probability. I've had people not turn up for interviews because they've got the dates mixed up and haven't held it against them. The fact they have let you go for another interview shows they are still interested, just be honest.

yes, just explain the truth. you understand that everyone makes a mistake now and then. you could tell a story of another time (a few years back) when you made such a mistake and how it turned out. but it's no big deal.
