"May I please change my lunch hour for just one day for a personal reason?"

You'd be telling the truth, only the facts, and the reasons why need not be said. Prefect for serious business people who wants to know that facts and nothing else.

You feel like you always have something to do? If you're asking this of him often, then I would be careful, especially if that sort of thing doesn't tend to go well. If this is just one occassion or it's rare, you just need to say you have an appointment - it might be too personal to say doctor's appointment, so you can say that it has something to do with your car, or the dentist. However, you probably don't owe him any sort of explaination and you're just being more careful than you need to be.

If there's a legitimate one time only situation then just tell the truth. If you can't tell the truth then you need ask what's so important that you would allow it to interfere with your work hours and even consider not telling the truth to participate in whatever it is.

Just tell him that you have an appointment at a certain time and could you take it for your lunch time. If he asks what it is say it is personal business you have to take care of but usually mine never asks.

Say you have to go to the doctor for something personal, or just ask him if there is anyway you can do to get it changed.
