I am trying to do a market research. I find sending a survey word doc by mail cumbersome.

Simply create your own site for the survey on any of the free sites (like Yahoo's own Geocities). There are several free services you can use to have the results of you form emailed to you.

depends on the crowd you're going after. There are a few "younger" based sites like www.quizzilla.com, just search for free online quizzes, if you search for surveys you'll get a whole bunch of those "take surveys for $$" places

why dont you use one of the free webhosting sites such as geocities,lycos ,bravenet ,google pages etc.

If you want a opinion poll try misterpoll.com,they allow you to create your own polls and give a tabular survey of the results
