They are called "Transfers-Papers", you can get them at most Arts & Crafts stores. Just place them delicately in your printer as they are quite soft. You can do them yourself with an iron or many tourist T-Shirt shops use a T-Shirt Press. They are usually around 18" square and you just place the shirt on the press, put the transfer paper on the shirt, and pull down the handle. Wait for a few seconds and it's done!! If your interested in a Press Machine, I have one to sell! By the way, always wash them in cold water, turned outside in and hang dry to help prevent the transfer from cracking!

If you're talking about the kind in flea markets, they're just big steam presses without the steam, but you can use an iron at home at a low setting.

It's an iron-on decal.
You print on a special paper (maybe with special inks).
Apply heat. The image from paper is transfered to the shirt.
If you want a bunch, then silk-screening is a good idea.
