Extremely high doses of time release niacin can be toxic to the liver. Extremely high doses of niacin are used by some doctors to treat high cholesterol levels. When doing this type of therapy the doctor should do two things:

1) use regular niacin and avoid time release forms

2) regularly check liver function with a blood test.

No. They simply allow water soluble vitamins to be excreted in the body at a slower rate instead of using it up faster.
