Every time my controller comes loose I have to remove power from the console and then hold A button down and start up with the controller. Sometimes this works the first time, and sometimes I have to try it several times. I can't find anything about this in the owner's manual.

I have a wired controller and when it gets unattached from the main cable, I plug it back into the wire and press the button in the middle which normally brings up the dashboard (The Green X) to let the 360 know I have plugged it back in and then press the "A" button which it asks for. I have had no problems doing it this way with wired or unwired controllers.

It takes a few seconds for the 360 to recognise the pad and then you should be able to press "A" and return to the game you are playing.

hold down the xbox dash button (the big grey button with an x on it) for a few seconds and then release, the xbox should reconnect it
