Well, unless your elbow is calloused, you don't have an effective scratching surface on your elbow :-)
The reason that you can't get your elbow up there is that the shoulder and muscles on the underside of your arm can only move so far, and it prevents the arm and elbow from flexing in that direction.

I had this trouble myself a year ago. I went to the doctor and he recommended a shoulder removal operation. Now my arm comes directly out of my torso and I can even scratch the middle of my back. AAHHHhhhhh relief!
