Many larval insects are technically the dirtiest looking. Many critters like leaf beetle larvae and aphid lions (lacewing larvae) will actually cover themselves in excrement or the dead bodies of their prey in order to provide themselves with camoflage.
Flies can be dirty, well more specifically the flesh flies and $hit flies. It all depends on what the fly is feeding on, if it is feeding on nectar, then no, but if they spend their time on rotting carcases and on feces, then yeah they could possibly be a huge vector for disease.

Red lily beetle larvae... they basically cover themselves in their own excrements...

They're pretty darn dirty! But are they dirtier than cockroaches?

i think they are.. they eat poop, then want 2 land on our food.. ewww
