Communication. Yes! You need to talk to her & find out the truth. The truth is always a truth, no matter what it is, you both have to live with it.
One thing I really don't understand is that, how can someone be so careless? Who will walk out without an underwear. Unless, the person was drunk, in the hurry or perhaps they want to be noticed & to be known. Or was it a joke? No many would like this kind of joke, I guess. Well, that you have to find out.

anyway, put them on her side of the bed on the floor so she can see them, put a camera in the room, and wait and see if she hides them, and have the camera in there the next time you have a buisiness trip

they could be your son's underwear. or maybe your daughter is screwing in your bedroom while both of you are gone and her boyfriend left by mistake. or maybe your son's gay lover, or maybe your wife misses you so much that she bought a pair of underwear to remind her of you. or maybe she left them out cause she found them and they are your gay lover. or maybe my final maybe they belong to the pool boy who cleans your pool and your wife wash them along with your underwear and gave him the wrong pair.

I think I would try to find out where they came from,,,, put them where your wife can see them and see how she reacts.
