First, you see a psychologist (or preferrably a psychiatrist, who is also a medical doctor) who sees this kind of patient.
Then you talk. Why do you think you want to change? There will be questions and tests. The shrink will let you know if you have gender dysphoria. If you don't, there is consultation to discuss the root of your concern or explanation of what sex change is all about and what it involves. If you are, there is a well-established set of benchmarks where the shrink and an endocrinologist helps you on the way or stops you if you can't tolerate the hormonal and lifestyle changes.
It often takes a couple of years and is very expensive. But first, it starts with a visit to a mental health professional.

Change their gender, or their sex? You must differentiate. If someone wants to change their sex (their bodies, hormones, and genitals), it's because they feel that's what they truly are. It's hard to understand fully unless you are in that situation. It's not that something really happens in the brain or soul- it's just gender dysphoria, when you suddenly feel a discord between your inner self and outer self.
If you want to change your gender (how you act and look on the spectrum of masculine and feminine), it has to do with your personality, how people react to you, your goals and all that- the personality part is fixed, and the rest is just a choice.

As the first answer indicated, gender is not the same as sex. Sex has to do with rep0orductive organs and reproductive capability, while gender deals with the way we accept ourselves and others relate to us. Most of the time, the two match -- sometimes they don't, and a person with the instincts of a man is forced to behave like a woman, or a person with the feelings of a woman is asked to act like a man. Writing about it, it sounds easy, like a party game, but over time it's a personal hell -- people with gender dysphoria are forced to repress all normal feelings and instincts, and live a life of pretend.
Changing sex through hormones and surgery liberates us, lets us follow our feelings, and frees both our minds and our souls to find their own way.

You can't change your gender. You can change your sex but not gender, gender is how you identify yourself. Sometimes people gender identify with something other than there actual sex and will go through procedures so that there sex and gender match up. The mind and soul stay the same, it's only the body that changes.

You don't have a soul. That's a myth. Homosexuality is a genetic defect in the human body which nature uses to reduce population of stop inferiors form breeding. It works well in animals as it is in the penguins now being studyed.
It doesn't work too well in advanced species of mamals such as humans. Their higher intelligence makes them more independent than the othe rmamals,and also culture and taboos hav e alot ot do with spreading gay genes.

Because the brain is physically female to begin with and the body male many doctors are now referring to transsexuals as inter-sexed and it is a recognized birth defect.
