Well it could be someone who has been diagnosed with one of the Psychotic Disorders i.e. Schizophrenia, Schizoaffective Disorder, or Bipolar Disorder/Major Depressive Disorder with Psychotic Features. Along with this Axis I diagnosis, they would be diagnosed on Axis II with Narcissitic Personality Disorder or Personality Disoder NOS with Narcissitic Traits.
i.e. The person would suffer from psychosis but would have delusions of grandure (believing themselves to be put here for something very important or to have a "higher" purpose in life)and would believe themselves much better than other people or perfect. (this is just an example and there could be many others)
