There should be a copy on your GP's file. If not, or for some reason they are not willing to show you, you should write to the NHS Trust concerned, and address your letter to the Records Manager. You are entitled to see any records kept about you under the Data Protection Act (DPA) 1998. Be aware that the hospital will probably charge you for providing a copy.

This website has a FAQ about accessing medical records: http://www.dh.gov.uk/PolicyAndGuidance/InformationPolicy/PatientConfidentialityAndCaldicottGuardians/AccessHealthRecordsFAQ/fs/en?CONTENT_ID=4039714&chk=iOJNGp#5145263 .

If, however, the hospital feel that anything contained within the report could cause you serious harm if you read it (for example, if you have a mental health problem which could be exacerated by reading the report), then the information with be exempt from the DPA and they will not be obliged to share it with you. http://www.yourrights.org.uk/your-rights/chapters/the-right-to-know/data-protection-act/health-records/health-records.shtml
