(I'm assuming that by asking how you spell "my name" is Spanish, you want to be able to say "My name is Tiana.")

What is your name? (formal): ¿Cuál es su nombre?

What is your name? (informal): ¿Cuál es tu nombre?

que es tu nombre (tu can be replaced by su depending if the question is formal or informal)
como te llamas (literally how are you named)
