A doctor cannot abuse the doctor patient priveledge. He will not say anything to another person about it. He can tell if you do it that way though. You'll show signs like tearing and swollen vains around the area.

He can, if there is severe tissue damage, tearing, bleeding, redness, or swelling. But what in the world would you be doing to cause that bad of a reaction ?
So, in most cases, I doubt it.
Give it a rest for a few days though, before you see the doctor.

If he checks, he could probably tell. Most likely he won't say anything because it is fairly normal. He is only looking for things that are not normal.
About the thing he could say would be to caution you to be careful about the size of the object, and to always use plenty of lube.
