This tool (no need to download it is all in the website) takes any picture and saves it in a .icon format that you can use on as a windows icon.
(dont go to websites that have the icons already made, most are filled with advertisments, and worse then that, viruses...)

Just use a photo editing software like Photoshop or Paint Shop Pro

If you want to manually edit a picture, you may use a free image editing program such as http://www.gimp.org and add text and other effects to it.
