First of all do not judge yourself, this is a sign of self importance and low self esteem. By allowing you to feel the question and asking yourself you will feel the answer. communicate what you hear in your heart. We already now all that there is to know because there is no part of us that is not God. So all we need to do is have the question asked and we can give the answers becuse the answers are in us. This is called inuition, meditation and willingness to be of service to Source. Be willing to feel the truth of the immensity of all that you are and allow you to be you by loving you. The only limitations we have are those that we allow to be placed upon us. "I beleive in me." "I unconditionally love me."

I don't know of any official questionnaires, but if you want to do it informally you can just ask your friends and relatives, "Do I strike you as being confident?"
According to some, those results will be about as accurate as any questionnaire.
