I believe I would try to go to the doctor and explain that you did have what you thought was a self-diagnosed ear infection and stayed home for two weeks because of it. It may cost you the price of an office visit but it may be worth it to save your grades. I would think twice before taking any more time off of school for any reason, even if you feel sick in the future. Let your teachers see that you are glad to be there and that you are going to be there, regardless. Maybe if they see you exerting some effort, they will be less likely to request a doctor's note next time. Good Luck.

That's too bad. The real problem isn't that you missed two weeks' of classes - although that's certainly an issue. The real problem is that you lied about your reasons why. And now you're stuck.
I don't know any doctor who'll give you a note like this. Do you know any doctors personally that you can ask about it? Someone in the family, or a friend?
It's too bad, because if you'd simply been honest - said you had some personal problems that prevented you from being able to attend class. Apologised. Said it wouldn't happen again. Asked if you could make up the work. And even if you couldn't, did well for the rest of the semester - you may have been all right. It would have affected your grade, but probably not as much as it will now.
As a professor, I've always tried to be understanding with students who are honest with me. And believe me, I can usually tell when you're spinning a story. And two weeks off for an "inner ear infection", especially without a doctor's note? Two weeks, and I never heard from you - not even an email?
But now, what do you do? If you can't get a doctor's note - and I don't think you'll be able to get one - you're in a bit of trouble.
The alternative, as another person wisely suggested, is to go to the doctor and explain the real reason why you skipped class. To me, and to the other poster, it sounds like you sunk into a bit of a depression, and may still be depressed. Is this possible? If so, that's something you CAN honestly speak to the doctor about. And you may be able to honestly get a genuine doctor's note, explaining the situation. And you could get some help.
I suggest you make an appointment with a doctor and discuss the entire problem - the personal problems, how they made you unmotivated, how you skipped class, etc. See if you can get a note related to the underlying issue, if there is one. The note, by the way, won't talk about your diagnosis. It will simply say that you were ill and missed class because of that. Your professors, unless you want them to, don't need to know the diagnosis.
Also make an appointment with the program coordinator or your academic advisor. Come clean with them, be honest. You had personal problems, and became unmotivated. You then panicked, and lied. You've since seen a doctor, and issues are being resolved. And here's the note...

You are depressed. Your physician will understand this if you explain and this will require no begging. Many people are embarrassed about emotional difficulties and choose to not reveal this. This is the truth and you can let your school folks know this. Your physician will provide a note explaining your depression and prescribe something for you. Also get a referral and get into counseling.
Stop worrying about your first explanation and get real with everyone. It will be a good first step for you.

You lied, you got caught. You should not ask your Dr. to lie for you. Taking a two week vacation because of "personal" reasons was your own decision, now you need to live with the consequences of that. You are lucky they are only dropping it by two grades. In the real work world you are fired after three days without a Drs. excuse.

You wont know what your options are until you talk to your instructor and let he or she know what happened. I would start with the instructor first and if you arent satisifed with the answer you are given then try your academic advisor or the dean of the school, to see if you can have time to makeup up the work.
