Where can I get financial assistance for Maxillofacial (Jaw) Surgery?

My insurance will only cover 60% of the surgery. That being only what they consider "usual and customary" charges... which they seem to be unwilling to disclose. Along with the surgery comes braces, consultations, and therapy. Total cost somewhere between $30K - $40K. I make less than $40K/yr. Somebody help!!

I just had this actually, and I am getting used to my new face. (Its weird not recgonizing yourself in the mirror).
