The main character (Guy Pearce) didn't kill his wife, she was killed by the burglars who also injured him during the break in. With the help of a detective, he tracked down the "John G." that murdered his wife and killed him, but couldn't remember the event after because of his brain injury, and so went in search of "John G." all over again.
The detective continued to help him in this quest, and use him, as he did with the latest "John G.", who was a drug dealer that Guy Pearce killed because he thought that he was the "John G." that murdered his wife, and the detective then took the deceased's drug money.
At the drug dealer's killing, the detective tells Pearce all of this, and the fact that even his initials are "J. G.", knowing that he won't remember the story in ten minutes, but Pearce notes the info and goes in search of the next "J.G.", which is now the detective.
Since the movie is told backwards, you see the detective killed at the beginning of the film, and the events of this last "John G." adventure unfolding afterwards.
"Sammy" was a con artist that Pearce investigated and exposed. Pearce's wife was the diabetic, and he administered shots to her. The Sammy accidentally overdosing his wife story didn't occur. It was a fabrication of Pearce's confused mind.
This film takes multiple viewings. It's confusing, but very interestingly made. I understand that some dvds actually allow you to run the story forward.

Interestingly enough, while there are those within the framework of the movie who take advantage of the main character, the truth is...
He has a condition that is entirely of his own creation. His wife left him...he can't handle that...so he makes up a story in his head that she was murdered...and everytime he gets close to revealing that truth...he destroys the evidence that led him there...
Sometimes that means he commits murder...but like Ahab's Whale...if he can convince himself that his wife was murdered and the murderer is still out there, his life will always have meaning...
The tragedy of Moby Dick is that what happens if Ahab actually killed the whale? What would he have left to live for?
In Memento, even though its false, he has a purpose in life...

I own the dvd and loved this movie. I understood the story exactly as John W here explains but it does leave you with the doubt of whether this is just the way serial killers think. The great think about Memento is that you can discover how lines in the dialogue make more and more sense every time.

I don't think he killed his wife but he did trick himself into thinking some other guy did so he could kill him. It was pretty confusing and backwards but watch it again and it will make more sense.

If I remember correctly, the black and white scenes are in order and the color scenes are in reverse. if you watched it that way you'd comprehend it. Sorry but I don't remember the details...
