Bad design, ugly people, pretentious, artyfarty, predictable, deliberately provocative, what's the worst piece of album art work ever? The New Order covers? The Doors? The Smiths? What's your opinion, my friends?

The ones that are the worst for me are the silly arty-farty ones that try and copy other 'classic' albums and fail miserably, like Coldplay's X&Y album. It is silly and tries to be high art, and is clearly a copy of the Joy Division album front for Unknown Pleasures. It fails, miserably.
I must also disagree with your criticism of New Order album covers- I am a massive New Order fan. I'll admit they aren't brilliant covers, but one of N-Order's strengths is that they can leave their music to do the talking, and not faddy album covers. Even so, the front of the Power, Corruption and Lies CD is still highly recognisable and different.

It's pretty difficult to mention just one cover, but I remember an one which I consider really poor: Up album from Right Said Fred band. Just because it only brought the name of the band in giant lyrics and the name of the album in very small ones. Quite stupid, huh?!
