Look for some kind of liquor dispenser. I found one that holds 2 bottles upside down and has little nobs at the ends that you turn to dispense the liquor, perfect for shots or mixed drinks. Also there's another that I found that you pour your alcohol into and it resembles a gas pump and has a small handle like one that you'd use to pump the gas that dispenses the liquor. I got them both for a friend last Christmas and he loved them.
Heres a website with the multiple bottle dispenser:

And here's the one with the gas pump:

What about the idea of buying a gift on his behalf for an under-privileged child in Uganda (or elsewhere in the Third World)? I am involved in a NI based charity which runs this scheme for Christmas or for any occasion such as a birthday. Your brother will receive a birthday card to say that, instead of giving him a gift, you have blessed a Ugandan orphan,You can check the details of the charity on www.rockministriesnitrust.org - there you will even find this gift idea though the gifts and prices are for last year as this has not been updated. If you are interested please contact me on rockministriestrust@hotmail.com

There's a chess/checkers game
that's characters are shot glasses
when my oldest turned 21
he and his friends had a lot of fun with it I hear
supposedly no one could play a game of chess...after a few shooters you're lucky to play checkers
prices $29-$15 (ebay)

I know I seem to recommend these to everyone - but how about those "once in a lifetime" gift boxes (racing cars, hot air ballooning, etc.). They're different from the usual CD's, DVD's, etc.
