Its not really possible to see much until about 6 weeks at the earliest. They probably wouldn't see much at 4 weeks, perhaps a yolk sac if the pregnancy was well established but nothing otherwise.
I had a transvaginal scan at 7 weeks and they could see the foetus quite clearly with the tiny heartbeat! He looked like a little shrimp, Heheheh!
I'm now 38 weeks pregnant and had a scan last Thursday and he looks like a full size baby (its a boy!)

You can see the foetus in the womb at approximately 5 1/2 weeks from the date of your last period.

The fetus itself, no. At four to seven weeks it looks just like an egg yolk. Nothing has formed and the cells are still dividing rapidly. By about eight weeks it begins to look like a tadpole, sorta. Then from there slowly the fetus begins to look more like a baby and not like an alien.
But if your asking can they tell your pregnant with an ultrasound at four weeks then yes because the look for the yolk sac! Good Luck and Best of Wishes.

I had an ultra sound at 5 1/2 weeks and all the doctor could see was the sac. When I was pregnant with my son I had a scan at 6 weeks and he was only the size of a grain of rice, but his heartbeat was very strong

I had a scan at 5weeks 5 days and we saw the heartbeat, I think they say that once your Hcg levels are at 1000 they expect to see something in the womb

No it wouldn't be visible yet. Ultrasounds are usually done at about 6 weeks at the earliest. If you think you are pregnant and are worried the best place to go is the hospital and check with them
