Here in Kyoto, there are no Geishas. You can still see Maiko, or Geishas in training, walking around sometimes. But, there are also a lot of companies here that will let girls dress up like a Maiko, and walk around for a day in full costume, so often what you see is not a true Maiko. They often have to carry a bag or something with the company's name, so that if they do something un-Maikoish, then everybody will know that they are not the real deal.
So, at least in Kyoto there are no Geishas left. I cant say for certain, but I'm 99% sure that there are no Geishas left in Japan.
EDIT
I'm beginning to think I'm wrong. But, that's what the Kyoto-ites I asked told me, so if I am, it's not my fault!

Of course. Actually there's quite a lot of them in pretty much every onsen (hot spring) resort. They are specifically called "onsen-geisha." They are facing the challenge of an aging workforce like most traditional industries do. Now the new word "companion" seems to be replacing the "geisha," especially when they are young and doing the job on a temporary basis. Whether companions or geisha, what they do is not much different, except that companions rarely wear kimono. They might be much more scantily clad than geisha to better satisfy customers.

Oh my f*cking god. For the last time, Geishas are NOT prostitutes! They are entertainers!!! Get your facts cleared up Steven Spielberg, and THEN write your goddam movies. That movie, "Memoirs of a Geisha", made SUCH A BAD IMPRESSION of Japanese Culture. It made it look like prostitution was a part of our Culture!!!
And in response to your question: yes. They still maintain their roles as entertainers, tehy do not exchange sexual favors for money.

Yes, they do exist in certain areas. It is more of an entertainer than anything else. As their clienteles are dwindling and other forms of entertainment widely available, I'd expect it will not be long before it becomes thing of a past.
