It seems like certain people like mathematicians, physicists and people of these fields get serious mental conditions like schizophrenia, and multiple identities. Is this just a movie thing or is there a real truth to this?

Brilliant people usually have irregular brains. This increases the likelihood of organic brain dysfunction.

Schizophrenia is a serious psychiatric disorder. It is most often diagnosed in the late teens and twenties. It is thought to be largely biological and there is a genetic component. Environmental influences also play a role. For most people it is a very debilitating condition. While there are famous examples (John Nash - from the movie A Beautiful Mind), of people who have achieved some greatness despite their condition, this is probably the exception, rather than the rule (but it makes a better movie). It is also true that very high intelligence is out of the range of "normal" so very smart people can also be quirky or eccentric. That's quite a bit different from having a serious mental conditition.
