There are two possibilities with that fan. First, there just might be a build-up of dust, so cleaning will fix it. The second is that the bearings are SHOT, and you need to replace it.

Look at NewEgg for a replacement:

Thermaltake CRYSTAL ORB (A1178) VGA Chipset Cooling Fan $8.99

ARCTIC Ceramic VGA Cooler $19.99

First off you have ATI and an AGP card. Your best bet would be to buy a new card. I purchased an ATI before and never again. That thing lasted 6months if that! I went with Nvida and have never had a problem with it ever and I have had my Nvida for almost a year and it's a sold product and trusted by most PC experts.

The only way to fix this problem is to replace the fan. Believe it or not, this is actually a common problem with video cards. If the noise is at a tolerable level, then there is no need to replace the fan.

Unauthorized, counterfeit or pirated video enjoying cards are periodically electronically zapped to disable them. This zapping immediately adjustments codes on valid enjoying cards and that they proceed to artwork, undesirable enjoying cards substitute into valueless, that are often offered from e-bay. possibly you in basic terms choose a sparkling video card fan.

Dirt / dust gets in them and then the bearings start going out and then they start making funny noises. I wouldn't try to fix it. You'll just make it worse. Buy a new one (fan, not graphics card). They're fairly cheap. Check eBay (my favorite place to buy stuff).
