All that is really necessary is to say something like..

Effective as of December 5. 2006. I am resigning my position as Clerk in the Accounting Department.

If you want to add more information you can, but that is all that is really necessary. Just make sure you give the necessary amount of notice and leave your old job with everyone wishing you well. Don't ever burn employment bridges if you can help it.

Try something such as this:

To: Human Resources
Date:
Subject: Resignation notice.

After careful consideration, I have decided to resign from Ajax Company effective on November 20, 2006.

I have enjoyed working at Ajax Company during the past two years.

A new opportunity has been presented to me by another company in a different industry.

I hope that I will be eligible for rehire from Ajax Company.

Regards,

You name and signature.

Honestly, I don't know what line of work you are currently in, but I would consider the pros/cons, before resigning in your current positon with your employer. Do you have another job lined up? At this time of the year it's very difficult to find another job depending again what you do for a living. At least give two weeks notice and don't burn any bridges for the future...

The ones I have seen goes like this :

'I,(your name), would like to tender my resignation with effect from (date). As required by the company, I am serving (one?) month notice. As such, my last day of service should be (insert date).

The above is the basic info required. U might want to add niceties like the first answer given too. ^_^
