I transfered to Cal Poly Pomona about 10-11 years ago and I did very poorly. My grades have improved considerably and now I would like to transfer to either CSU Dominguez Hills or UCLA. Do I need to include the transcript from CAL Poly when applying to these 2 schools? Will they know if I don't tell them?

You are supposed to send transcripts from all the schools you attended, but it's not like CSU DH or UCLA will verify whether you actually did submit all the transcripts (they receive hundreds of thousands of applications every year, it's impossible to verify that kind of information ... and no, they are not all linked by a computer network). You do, however, sign your application stating everything in your application is true and correct, meaning you are technically lying to DH/UCLA by not including the transcripts. But I really doubt they will give you a hard time over a transcript that's 10 years old. Most of the time, anything older than 10 years is worthless anyway. You can call the admissions office and ask whether you need to include a transcript that's 11 years old and if the answer is no, then you are home free.

Accredited Colleges share transcripts and honor almost all of each others classes IF fairly recent.

There is a possibility that they wont accept all of your transcript due to timliness, but you have to take the classes over, and pay again. Some schools have a policy of not accepting incoming classes after 10 or 15 years, so check both schools policies; It may or may not be an issue.

The worst thing you can do is to lie about your old school or transcript, it can cause some serious ramifications at your new school when caught.

If they do send the transcript, and you decide to take the class again and do very well, you can petition your new school to remove the old grade from your record- it happens .

The UC campus are particularly greater prestigious and examine-oriented than maximum CSU campuses. You will need to send transcripts from each school you attended. They will notice that you improved and give you credit for that.
