It does hurt. I think most girls aren't incredibly open when the guy asks "should I stop" during intercourse. Most girls I've talked to said they told them to continue, even though it hurt. I think we do this because we tend to think the pain will subside after we get used to the foreign object penetrating our vagina the first time. And it does subside, with extra gentleness and patience of the male partner.

Yeah, it usually hurts the first time because of the hymen getting ripped. It shouldn't hurt her like that in the future. But keep in mind you can still cause her pain if you thrust in the wrong direction or try to penetrate to fast.
Ask her to talk to you about it. If you want to have a healthy intimate relationship then regular, honest conversation needs to be a part of it.

Women will generally inform you if they'd experienced some discomfort during intercourse; however, I would simply ask again just for reassurance. Further, you will want to be as respectful and kind as you can while inquiring.
