Well not me personally, but when I was a navigator in R.A.F. During air show my pilot made an error and we had to eject. Result loss of one 75 million quid aircraft. Plus numerous broken bones.

Odd for this category, but I'll offer this,,,also not at all related.
Expensive, in this case, isn't accurate,,, Of Great Value is.
Accident may also not be strictly accurate.
Damage however,, and levels of Breakage do apply.
Aldara, my love, I did damage what we had/ have,,, and will work to repair that, every day of my life hence.

TVR Griffith, driveway gate caught in the wind and chipped the gel coat on the drivers door, whoops. Then I sold that and bought a TVR Cerbera, drove over a raised manhole cover in a garage and ripped a massive hole in the chassis, even bigger whoops.

I don't buy expensive things purely for the fact that I am clumsy!
I did once throw a bucket and my brother, but he dodged out of the way and it smashed a glass door - that was about £150 to repair! Lesson learnt - don't throw things at your brother, hit him instead :)

I think I broke my first car. It was a Talbot Samba, with six previous owners. I think I overfilled the oil sump, after which the engine was never the same again. Ended up scrapping it.
