You need to keep in mind that filing for disability is more than just filling out a few papers. a better use of your time would be to call 1 800 772 1213 and make an appointment for your local office. they can be quite busy and you will often wait 45 minutes just to be given an appointment. no sense in leaving the house for that. make an appointment for disability and be sure that you have an original or certified copy of your BC.

Call your local Social Security Office. And keep in mind that usually people are turned down a couple times before they get it. If you can find an attorney who will take your case before you're even turned down the first time, that would be your best bet.
Oh, and hopefully your doctor agrees with your disability.

Same as any other state. Go to the local SS office and tell them what papers you want and they give them to you, then you fill them out and you will have to have your SS number, income, anything you own or have given away in the past few years, any assetts and liabilities you have, what the disability is supposed to be, etc.
