In our Order, American Co-Masonry (Freemasonry for Women and Men), the title is the Most Sovereign Grand Commander. She lives in Larkspur, Colorado. Her (or his when it is a man) job is to hold Freemasonry to the precepts and standards set forth in our bylaws and constitution. She also rules on matters having to do with ritual and matters concerning protocol within the Lodge if and when necessary.
We also have a President who takes care of mundane matters having to do with legal affairs and monetary things.
Although I am but a third degree, I know both of these people well, trust them, would offer them a kidney if either needed one of mine. What's more, they know me. Pretty special folks with lots of responsibility.

There is a "Top Freemason" for every Grand Lodge in the US. Britian has a Royal connection with their Grand Lodge but I'm not sure how that works. There is no one man that controles all of Freemasonry.
