Well, all of the artists that would be considered "underground" to the rest of the world (Little Brother, Nicolay, J Dilla, Madlib, MF Doom, Fat Jon, etc.) are pretty mainstream in our house and among our friends. But nowadays to most people, "underground" is just anybody who doesn't have their songs playing 2 million times a day, every day for months on popular radio stations. I guess variety in hip hop has become an old-fashioned novelty on mainstream radio.
As for my definition of hip hop (since I assume you're talking about just the music as opposed to the actual culture), real hip hop stays closer to the roots of hip hop music (i.e. - speaking on social and/or political issues, storytelling, bragging about one's lyrical skills while roasting another MC, etc.). And the producers know that there's a world beyond old-school, sped-up soul records when it comes to sampling (i.e. - jazz, rock, folk music, classical, etc.).
Just one girl's opinion. :)

Underground hip-hop might refer to hip-hop artistes who do not publish their works with the mainstream music producers. Such works created by unknown yet innovative and talented 'producers' is known as underground hip-hop - I think!
