Unfortunately, even death does not allow you to break a lease. I would talk to the manager of the apartment complex and explain your situation, and see if you can just pay a 30 day notice and the cost of reletting (usually 85% of one month's rent), and see if they'll keep it off of your credit. If you really are going to die though, why would you care about your credit anyway? Not only that, but this Doctor that has determined that you are going to die, did they give you an amount of time before this will happen? And if so, how do they know? Some people have lived WAAAAY past their life expectancy, and have thus proven to the Dr. that they are not GOD!!!
Good luck though!

supply a doctors or hospital notification and the other reasons behind it.
Got to move back home for care or closer to the hospital for treatment and such.
Start the letter with I am sorry to notify you that I find it neccesary to terminate my lease with your company. I am in failing health and find that for purely for medical reasons I must move home or closer to my health care. Please see attached verification of my illness. I am hoping you can see to allow me to break my lease with no penalty. I need to move Dec xx xxxx or as soon as I can.
Please let me know of any additional fees I may be held responsible for
