She realises that he was the one who died that day, because she had told him about that event after it happened in 2006- but still before it had happened in his time.
This prompts him to go to see her because in telling him, she has given him a day/time/place where she will definitely be when 2006 comes round for him.
But it turns out he was the one who died, as he was coming to see her. When it happened in 06 she hadn't started the letters yet, but he had been getting them for almost 2 years. The whole situation was basically created from her telling him about something in the future, which causes it, before she knew who he was.
So she sends him a letter and it arrives just in time in HIS 2006 (her 2008), to stop him from going. As such, history is changed, he is still alive, and then in 2008 he knows where she will be that day/time (by the Lakehouse, sending him the letter) and he waits and shows up the instead.
