Two ways. First, there is padding in the cups that lift the breasts higher. Second, the position of the cups will usually push the breasts toward the center, accentuating the cleavage. Some work better than others.
I recommend going to a Victoria's Secret store. They have a box for each size with try-on bras of all of their styles.

I worked on lingerie department for four years and was a qualified bra fitter!
A push up bra is only half of the cup its meant to be and the way its cut forces your breasts to push together and hence giving you a cleavage!
All the different bras give u a different look and work in a different ways...amazing at how much technology goes into it!

They are made with a slightly narrower cup, and they have a layer of padding in the bottom half of the cup. The padding helps lift the breast up, so that it sticks our farther than normal, and the narrower cup squeezes it together to maximize the 'lift' effect.

im not sure but i think that the extra padding at the bottom of the cup pushes up boobs because there is no space for your boobs so they have to go up lol
