Tolkien was enamoured with mythology, especially Norse mythology. His great goal was to write a combined mythology that would not be allegory but true myth making. As a matter of fact Gandalf's name in Gondor was "Mithrandir". Tolkien derived Middle Earth from the Norse "Midgard". Top Earth would be comparable to Heaven or to the Norse "Alfheim" or Elfhome. "Valhalla" was Odin's Hall where warriors go after they die in battle (which could be considered heaven for some). Bottom or Lower Earth would be comparable to Hell or to the Norse "Niflheim" ruled by "Hel".

Well in Tolkein's world yes. . The world of Arda which is the entire planet consisted of
Middle earth in the middle. And resembled most of europe and africa
Theres the undying lands which are to the left
Hither lands to the south
Aman, dark lands and Numenor were also in arda
these basically became the continents of today
Eventually middle earth took up most of arda as the undying lands left arda into space.

The forming and eventual demise of Arda (which includes Middle Earth) are defined quite nicely in the Silmarilion by Tolkein. Perhaps you could simply read it?
