Hi Kim,
Are you working on your care plans for school? If you have a surgery patient you will have many nursing diagnosis potentials. Since you asked about infection I will stick to that:
Infection: potential/actual related to surgical procedure.

asses pt for sign and symptoms of infection:
elevated temp. greater than 101 degrees (or whatever hospital protocol is)
redness, swelling, drainage at incision site.
rales, rhonchi, productive cough (what color is the sputum)
keep incision site clean and dry
give medications as ordered
contact physician immediately for s/s of infection
teach family/friends s/s of infection
goal: pt will have no infection postoperative through(establish a time line usually the estimated time of confinement.)
Most of the s/s of infection are easily recognized postoperative. Hope this helped. You know that with infection your main indicators are the elevated temp. , the lung sounds, and the actual appearance of the op site.
