It means that someone who is "great" is "greater than most other people", so they see things differently. Because of the difference, other people may misunderstand what their intentions are. For example, if someone is a great leader, someone else may think they are doing it just for power, even if the intention of the "great leader" is to do something good.

Sometimes, if you are the only one who gets a joke, you are thinking on a higher plane, and that qualifies you for an "elite" group. If you think like everyone else or if everyone understands you, how can your thought be distinguished enough to be unique?
